When care needs to start fast, the worry isn't paperwork, it's the basics getting missed in the first days and nobody spotting the change until something goes wrong. A caregiver proposal gives the family one written plan to agree on before the first visit, so everyone knows what's happening, who is coming, and how updates work.

The proposal opens with Summary, then introduces the people involved in Who you are working with, The team, and Team, before it gets into what care looks like in practice in What happens and when. The teal and amber layout under Raleway headings keeps the schedule and decision points easy to scan, and the later pages answer the questions families ask when a stranger is coming into a flat, including stairs, consistency, and how issues get flagged.

  • Summary Frames what gets kept consistent, how visit notes work, and what gets flagged early so the family isn't finding out late.
  • What happens and when Walks through the Erstbesuch and week-one rota, then explains how the ongoing visit routine and updates work.
  • Plans Holds the weekly pricing for Regular support, which you replace with your own rates before sending.
  • Next steps Puts acceptance at the end, with a fee summary and signature so the start can be approved and booked.
  • Payment and Visit notes Spells out weekly invoicing, access and key handover, schedule changes, and what gets documented after every visit.

We went from spending hours on every proposal to creating fully customized ones in under 5 minutes. That's not an exaggeration - we timed it.

The proposal in full

Summary

You know who is coming, what happened on each visit, and what we are watching for next, so care stays steady and you are not finding out late.

Most families call us when the hospital or Hausarzt says “she can’t manage alone”, and the big risk is missed basics in the first days. You’ll have a start date, a written plan, and visit notes you can read without chasing. Next, we’ll book the Erstbesuch and confirm who will be at the door.

We start with an Erstbesuch to check needs and day-to-day risks, then we set a week-one rota and sort keys and access. From the first visit onward we follow the same routine each time, write notes before we leave, and flag changes the same day so small issues do not sit quietly for a week.

Who you are working with

We’re a small in-home care team, and we built our way of working around the first week, because that is when most preventable problems happen. Anja Richter coordinates the start, the schedule, and family updates. Markus Vogel and Derya Yilmaz handle day-to-day visits, with the same visit notes system every time so nothing depends on memory.

We get picked when families need care to start quickly and stay predictable. We plan around real flats, real stairs, and real keys, so the caregiver is not improvising at the door. We also write down what we see, in plain language, after each visit. If something changes, we say what changed, what we did, and what we recommend next.

The team

You’ll deal with the same three people throughout, and you’ll always know who to call for what.

Team

Anja Richter, Care coordinator. Anja books the Erstbesuch, sets the rota, and is your point person for schedule changes and weekly updates.

Markus Vogel, Senior caregiver. Markus leads personal care visits, mobility support, and safe transfers, and he keeps visit notes consistent across the week.

Derya Yilmaz, Caregiver. Derya handles day-to-day support like meal prep, hydration prompts, and medication prompts, and she flags small changes early.

Recent projects

These are recent in-home caregiver support starts where speed, access, and steady communication mattered as much as the hands-on care.

Portfolio

Post-fall week-one start. We started with an Erstbesuch, then ran two visits a day for the first week while mobility settled. We set up a simple hydration and meal routine and tightened it after we saw what was actually getting eaten and drunk.

Medication prompts setup. We took over medication prompts and notes after a missed-dose scare. We agreed the exact prompt wording with the family, documented each prompt, and phoned the same day when the routine was not followed.

Altbau stairs care visits. We supported a client in a walk-up flat where stairs were the main barrier to leaving home. We planned transfers, pacing, and timings around the stairwell and kept the same arrival window so building access stayed smooth.

“They started fast, showed up when they said they would, and kept me informed.”

What happens and when

We run in-home caregiver support as a steady schedule, with written notes after every visit and one clear point of contact between visits. Most schedules start with a needs check, then settle into a repeatable weekly rota.

1. Erstbesuch Once, at start Anja visits the flat to check needs and risks, including mobility, washing and dressing, continence support, and medication prompts. We also cover access, keys, and stair details so the first care visit starts smoothly, not as a door problem. 2. Week-one rota First week We set the visit times for the first week and confirm who is assigned. We agree the practical basics that prevent missed care, like where supplies are kept and what “good day” and “bad day” look like. You get the schedule in writing. 3. Care visits Every visit We arrive, follow the agreed routine, and adapt within the plan when the day is different. We write visit notes before we leave, including what was done, what was observed, and what we will watch next. If something is off, we contact you the same day. 4. Update and changes Each week After the first week we do a family update call to confirm what is working and what needs to change. When care needs shift, we do a handover between caregivers and update the schedule, so the next visit reflects the new reality, not last month’s plan.

Initial assessment visit

One Erstbesuch at the start to check needs and risks, confirm access and keys, and agree the week-one plan and visit times.

CARE PLAN AND SCHEDULE SETUP

A written care plan and rota setup in the first week, updated when needs change so the visit routine stays current.

PERSONAL CARE VISITS

Every visit, as scheduled. Washing, dressing, and continence support are done to the plan, with adjustments noted in writing.

Medication prompts and notes

Every visit where prompts are part of the plan. We record whether prompts were completed and flag missed or unusual outcomes the same day.

MEAL PREP AND HYDRATION SUPPORT

On scheduled visit days. We prepare simple meals and prompt fluids to the plan, and we note patterns like poor appetite or low intake.

MOBILITY HELP AND TRANSFERS

Every visit where mobility support is needed. We assist with safe transfers between bed, chair, and toilet, and we note changes in steadiness or pain.

Plans

Choose based on how many hours of in-home caregiver support you need each week. We can start with one plan and move up or down after the first week. Prices are per week.

Priced items

Next steps

If you want us to start, we’ll lock in the Erstbesuch and send the week-one rota right after.

1. Sign to approve in-home caregiver support. 2. Confirm the best contact number and key handover plan. 3. Book the Erstbesuch and set the week-one visit schedule.

Fee summary

How fast can you start, realistically?

We can usually schedule the Erstbesuch quickly, then start visits as soon as the rota, keys, and access are sorted. After we speak, we’ll tell you the first available assessment slot and the earliest realistic first-care-visit date.

WHO EXACTLY IS COMING INTO THE FLAT, AND WILL IT BE THE SAME PERSON?

Anja coordinates and stays your point of contact. Markus and Derya deliver most visits. We aim for consistency, and we tell you in advance when a different caregiver is scheduled, including the reason and the handover plan.

Can you handle stairs in an Altbau with no lift?

Yes. We plan for stairs during the Erstbesuch, including entry, pacing, and what we can safely carry. If the stair setup changes what we can do on a visit, we’ll say that plainly and adjust the plan with you.

HOW DO YOU DOCUMENT VISITS AND TELL ME IF SOMETHING’S OFF?

We write visit notes after every visit, covering what we did and what we observed. If something is off, like a fall risk change, confusion, or missed medication prompts, we contact you the same day with what happened and what we recommend next.

Payment

We invoice each week of in-home caregiver support at the start of that week. Nothing is paid up front beyond that. Every invoice is payable within 7 days of its date.

Erstbesuch. The Erstbesuch is the first booked visit. We use it to check needs and risks, confirm stairs and access, and set the week-one rota. If the situation has changed, we adjust the plan.

Access. Before the first care visit, we need working access to the flat. We agree key handover and any entry steps during the Erstbesuch. For safety, please don’t send keys or codes in writing.

Rota changes. We build a week-one rota, then keep it steady where we can. If we need to swap a caregiver, we tell you in advance, explain why, and share the handover plan for that visit.

Visit notes

We write visit notes after every visit, covering what we did and what we observed. If something is off, we contact you the same day with what happened and what we recommend next.

Stairs and carrying. We can work in walk-ups and plan for stairs during the Erstbesuch. We pace visits around safe entry and transfers. If stairs limit what we can safely carry on a visit, we will tell you and adapt.

Extra time. If a visit runs longer than planned, we will use the time where it is needed and note why. Time beyond your plan hours is billed separately at our current hourly rate unless we agree otherwise.

Ending care. You can end or pause in-home caregiver support with notice. We’ll confirm the notice period in writing when you start, then invoice only for the weeks that fall inside that window.

Questions about this proposal template

What should a caregiver proposal include?

A caregiver proposal usually includes the care plan, how visits get scheduled, who is responsible for updates, and the weekly price. This proposal also covers access and keys, rota changes, visit notes, medication prompts, and when the family gets contacted the same day.

How do families get updates about each visit?

Visit notes get written after every visit, covering what was done and what was observed. When something is off, the proposal says the family gets contacted the same day with what happened and what is recommended next.

How fast can in-home care start after someone leaves hospital?

A care start depends on booking the initial assessment visit and getting access and keys sorted. The proposal says the earliest realistic first-care-visit date gets confirmed after that assessment slot is set.

Will the same caregiver come each time?

The proposal describes one coordinator as the point of contact, with day-to-day visits handled by the caregivers. The wording also says the family gets told in advance when a different caregiver is scheduled, including the reason and the handover plan.

How do you handle stairs in an old apartment building with no lift?

The proposal says stairs get planned for during the initial assessment visit, including entry, pacing, and what can be safely carried. When stairs limit what can be done on a visit, the plan gets adjusted and explained plainly.

When and how does payment work for caregiver visits?

The proposal states weekly invoicing at the start of each week of care, with invoices payable within 7 days of the invoice date. The terms also cover extra time beyond plan hours being billed separately unless agreed otherwise.
